Your First Visit

Thank you for choosing Apex Physical Therapy as your physical therapy provider.

We are honored to be an expert member of your healthcare team. We specialize in injury prevention, pre- and post-surgical rehabilitation, sports medicine and pelvic floor. Our therapists as are your full body experts, bringing balance and wellness to your body. New Patient Forms

We encourage all new patients to come to their first therapy visit 15 minutes early with their new patient forms completed. For your convenience in downloading these forms, please click on the link below.

Items to Bring to Your Appointment

  • Completed new patient paperwork
  • Your most current identification card
  • Your most current health insurance card
  • Any x-rays, MRIs or physician reports related to your injury (if applicable)

What to Wear

To best prepare for your upcoming physical therapy sessions, we ask that you wear comfortable, loose-fitting clothing. Basic workout or athletic attire is most appropriate.

What to Expect

On your first appointment, you will meet your physical therapist, discuss your current condition or injury, and identify your goals for pursuing physical therapy. The physical therapist will perform an initial evaluation (45-60 minutes). At the end of the initial evaluation, your physical therapist will present his/her exam findings and present a unique customized recommended treatment plan to improve your condition and help you reach your goals. We understand every patient is different and has different needs and conditions, so it is extremely important to realize that treatment plans will be unique, too. Please ask as many questions as you’d like as your therapy team is here to help you fully understand your treatment plan as well communicate with your healthcare team.

Appointment Cancellation Policy

Keeping all appointments is an integral part of helping you reach your goals and to get better faster. If you should need to cancel an appointment, please be aware of our requirement of at least 24-hour notice. Failure to cancel your appointment with more than 24-hour notice will result in a $50.00 fee. You will be responsible to pay at your next visit.
